We will endeavour to deal with your query as quickly as possible, however we have had to extend the expected completion time frame for this type of request to within 5 working days during opening times.

Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
If it is not possible to raise your complaint immediately, please let us have details of your complaint within the following timescales:
- Within 12 months of the incident that caused the problem.
- Within 12 months from when the complaint comes to your notice.
