Long-Term (Chronic) Disease Clinics

Our Nurses run Long-Term (Chronic) Disease Management Clinics, which are available from Monday to Friday. Times are variable between 8.20am - 6.30pm.

The aim of all our long-term disease management clinics is to work with you and your family members to keep you as fit and healthy as possible. Referral back to a doctor will be made if necessary. Regular blood tests are essential for patients taking certain medication(s).

Asthma - all asthmatics are encouraged to attend the Asthma Clinic annually for advice, discussion and a review of treatment(s). This is an important part of your self-management plan and ongoing well-being. More frequent attendance is available as necessary. These clinics are run by our Practice Nurses.

Chronic Obstructive Pulmonary Disease (COPD) - COPD Clinic is run in a similar way to the Asthma Clinic. Patients are encouraged to come to the practice for an annual review for advice, discussion and review of treatment(s). These clinics are run by Practice Nurses.

Chronic Kidney Disease (CKD) - most people with CKD will be attending one of our other chronic disease clinics but we do have a small number of patients who need an annual review of their blood pressure, blood tests and discussion on lifestyle who do not attend another clinic. our Practice Nurses will offer an annual review to this group.

Diabetes - The Practice Nurse can offer advice and discussion on diet, lifestyle and treatment plans. An annual review is recommended with more frequent attendance as necessary.

Hypertension - The Practice Nurse can offer regular reviews of blood pressure management and lifestyle advice.

Vascular - our vascular clinic sees patients who have had one or more of the following problems - heart attack, stroke, intermittent claudication, atrial fibrillation, heart failure or other vascular events. Our Practice Nurse offers advice and discussion on treatment, diet, exercise and lifestyle.
